*General Definition and Scope of Job:*
A clinically competent Licensed Practical Nurse who functions as a case manager and delivers direct patient care in the home setting.
The LPN delivers quality care through developing, implementing and continuous evaluation of an individualized plan of care.
Effectively interacts with the patient, caregiver, and other health professionals while maintaining the standards of professional nursing practice.
Provides clinical supervision to home health aides.
*Job Status: Casual * *What are the Minimum Skills, Experience and Educational Requirements?* * Graduate from an accredited or state approved school of practical nursing * Current licensure in the state of practice * Must have a minimum of one (1) year experience in an acute care setting.
* Home healthcare experience preferred.
* Must possess interpersonal skills sufficient to interact effectively with clients and their families as well as all interdisciplinary caregivers, peers, subordinates and supervisors.
* Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.
* Must have current Driver’s license and reliable transportation to and from work site.
* Ability to consistently demonstrate the Organizational Core Competencies and Organizational Code of Ethics, and adhere to the Compliance Program.
* Valid license as an Licensed Practical Nurse in Michigan.
* Valid Michigan driver’s license and automobile insurance * Current BLS certification *What are the Essential Job Functions and Responsibilities?* * Effectively and efficiently plans visit content and schedule based on client capabilities, condition, preferences (when possible), physician orders and available resources.
* Maintains a working knowledge of applicable Federal.
State and local laws and regulations, the Organizational Integrity Program, Code of Ethics, as well as agency policies and procedures and adheres in a manner that reflects honest, ethical and professional behavior.
* Performs specific treatments and administers medications to implement the medical and nursing care plans.
* Applies appropriate preventive, therapeutic, and rehabilitative nursing procedures and techniques to clients.
* Utilizes interview, observation and evaluation in assessing clients and applies nursing judgment, consistent with practice standards, in making recommendations to the primary nurse and client.

* Reports changes in client condition as appropriate and in a timely manner, to the client’s physician and/or Case Manager/designee, and obtains orders for changes in the plan of treatment to respond to the client’s condition.
* Instructs family members in client care.
* Assists in care plan revisions as indicated.
* Participates in care conferences with other members of the health care team.
* Utilizes and implements appropriate tools and pathways per guidelines and/or policy and procedure (e.
g.
education checklists, pathway protocols, Home Program Instructions, etc.
).
* Participates in care conferences with other members of the health care team.
* Accurately documents nursing observations, interventions and evaluations on day services are rendered.
* Submits/transmits documentation in a timely manner.
* Works with scheduling to assure accurate and up to date schedule.
* Establishes and maintains effective communication regarding the client’s care with client and family members.
* Keeps current in area of expertise and assures that standards and practices within the agency reflect best practice within area of expertise * Acts independently and responsibly to perform duties on a consistent basis and in a timely manner.
