Summary:
Under the direction of the Medical Director, the Nurse Practitioner provides medical care and education to patients and families in a variety of settings, inpatient, outpatient, home, and facility based, for both non-hospice palliative care (Keystone Medical Services) and hospice patients (Hospice of Holland).
The salary range for the Nurse Practitioner is between $98,880 and $126,880 annually, with a sign on bonus.
Responsibilities of the Nurse Practitioner:
Provides education to patients and families.
Provides patient visits to palliative care patients, in both inpatient and outpatient settings.
Provides patient visits to hospice patients, in both inpatient and outpatient settings.
Provides patient telephone advice to palliative care patients during normal business hours.
Participates in Interdisciplinary Group (IDG) meetings.
Presents education to the community regarding Hospice or Palliative Care, as opportunities arise.
Assists in Palliative Care program development through excellent customer service to referral sources, patients, families, and peers.
Performs face-to-face visits to determine continued eligibility for hospice programs.
Develops and maintains effective and positive relationships with health care professionals within the community.
Understands and assures compliance with local, state, and federal regulations and standards associated with the care of hospice and palliative care patients.
Participate in weekend and holiday on-call coverage.
Become credentialed to provide care at partnering facilities within four months of hire.
Obtains Certification in hospice and palliative care (CHPN) within 1 year of hire.
Qualifications for the Nurse Practitioner:
Graduate of an accredited Nurse Practitioner program.
Current State of Michigan Registered Nurse and Nurse Practitioner Licensure.
Minimum of three years of experience as a professional Nurse Practitioner or equivalent clinical experience.
Understands the philosophical and technical aspects of hospice and palliative care.
Demonstrates a knowledge base in technology of palliative medical management especially pain and symptom management.
Excellent communication skills and proficient with mobile devices (iPads) and electronic medical records and computer software (Microsoft Office, etc.
).
Medicare and Medicaid Provider in good standing.
Current State of Michigan driver’s license.
Reliable cell phone with local coverage.
