Position Overview Under general supervision of a Mental Health Team Supervisor, conducts assessments; develops treatment plans; and provides community based, mobile treatment using a broad range of services for individuals with severe/persistent mental illness, or co-occurring mental illness and substance use disorders.
Essential Job Functions The essential functions of this position include, but are not limited to, any combination of the following: Provides services to consumers, mainly in their homes and in the community.
Services may include, but not are limited to, medication monitoring, crisis intervention, assistance with addressing basic needs, some case management, assistance with setting up/taking medication as prescribed, and providing injections of prescribed medications.
Conducts comprehensive, quality assessments of both mental and physical health needs.
Provides input into the creation of multi-disciplinary, comprehensive, quality, individualized treatment plans.
Assists in monitoring consumer medications and lab work by reviewing and interpreting physician orders for medication and lab work and provides support and education for non-medical team members in assuring orders are carried out correctly.
Provides support, as needed, to the team psychiatrist, by coordinating with other medical staff to, but not limited to, taking vitals, communicating medication changes, creating lab orders, and performing other medical functions as needed.
Administers IM medication as prescribed.
Injections are frequently provided in the consumers' home or in the community, and occasionally in the office.
Works with other medical staff to provide monitoring of medications, including but not limited to, making sure medications are refilled, setting up medications as needed, and providing support and education to non-medical team members about medications.
Monitors consumers' physical health care needs, links consumers to community health services, and provides education and direction to other team members about consumers' physical health condition and recommended treatments.
Attends team meetings and other meetings as needed.
Maintains work level within established agency productivity level.
Completes required paperwork in an accurate and timely fashion.
Maintains positive community contacts.
Maintains and improves working relationships with community agencies, medical offices, and pharmacies to ensure coordination of services and medications.
Provides after-hours on-call coverage for ACT/IDDT consumers in crisis.
This position is required to take one weekday per week, and one weekend per month, or more shifts if desired.
Serves as a resource and to answer medical questions for non-medical team members.
