The Care Manager RN PHP/IOP (Partial Hospitalization Program/Intensive Outpatient Program) is a central component of the Mental Health PHP/IOP multi-disciplinary treatment team.
This position is responsible for the nursing assessment and treatment of patients in the PHP/IOP level of care.
The Care Manager RN will complete an initial assessment and assist in the medical management of patients in the program in coordination with the physician.
The contacts third party payers with clinical information for approval of patient days in the programs.
Additionally, the Care Manager RN PHP/IOP will participate in service delivery via groups and classes which promote healthy lifestyle, skill utilization and symptom improvement.
The will participate in continuing education, and will comply with department specific competency requirements.
Pay Grade: $28.

Patient Care-Assessment and Treatment Completes timely and accurate nursing assessments and continually assesses the medical needs of patients involved in the program.
Coordinates with the program psychiatrist to address prescription questions, concerns and refill needs Provide psychoeducation and skills training, both individually and in the group/class format through evidence-based treatment modalities.
Continually assesses patient’s status and progress toward treatment goals.
Is alert to safety concerns and recommends appropriate levels of interventions based on patient needs.
Facilitates medical aftercare planning to ensure ongoing needs are met following patient discharge from the PHP/IOP level of care.
• Patient Care-Documentation Documents assessment, education and treatment in a timely, accurate manner that is in compliance with regulatory requirements, policies and procedures.
Completes required nursing assessments, group/class notes, progress notes and case coordination notes using symptom-focused, behaviorally-specific language.
• Utilization Review Contacts third party payers to obtain authorization for program days for patients.
Communicates severity of illness and intensity of service criteria.
Acts as a liaison to hospital financial offices and facilitates Medicaid or charity applications as necessary.
Maintains ongoing records of days approved or denied.
Files retrospective requests for payment or appeals for days denied.
• Functions as a Professional Member of the Multi-Disciplinary Treatment Team Attends and participates in team meetings.
Works collaboratively with team members, seeking and incorporating their input.
Promotes interpersonal team relationships by giving/receiving open and direct feedback among team members.
Maintains professional licensure through accumulation of required CEU's.
Attends staff education meetings and meets mandatory department unit competencies.
