The RN is responsible for the provision and coordination of patient-centered care.
The RN will assist the physician with patient calls, maintaining medical record information, coordinate patient care with other health care providers including necessary prior-authorizations.
The RN will focus on prevention, health promotion, patient education, and patient assistance.
Under the supervision of a physician the RN is involved in providing telephone assistance to advise, counsel and instruct patients on health care treatment for acute and chronic conditions.
PROFESSIONAL NURSING PRACTICE
Promotes patient safety by maintaining the policies and procedures of the physician office.
Achieves office-specific initial and ongoing competencies.
Maintains the ANA Standards of Professional Performance including: Participates and is mindful of the quality initiatives of physician practice; maintains nursing practice in relation to professional practice standards and guidelines, relevant statutes, rules and regulations; and contributes to the professional development of peers and colleagues.
Works with physician and patient rendering medical care via telephone.
Coordinates patient care involving other members of the health care team in relation to Patient-Centered Medical Home as appropriate.
Considers factors related to safety, effectiveness, cost and impact on practice in the planning and delivery of nursing service.
Works with other team members to ensure appropriate access to patient care.
May review incoming referrals to ensure they are appropriately triaged for urgency.
Provides patient education either in person or via phone regarding upcoming procedures in the office or hospital.
DEVELOPS AND IMPLEMENTS NURSING REGIMENS
The RN may develop and initiate nursing regimens, for example: Assessing patients via phone to determine need for treatment or urgent/emergent care.
Will use standard protocols developed by the nursing team and approved by physicians where applicable.
Implements patient management plans as devised and directed by a physician.
Orders medications as directed by a supervising health professional.
DOCUMENTATION
Documents patient information in the medical record.
Assists with maintaining patient medication logs, problem lists, completes prescription files and communication logs.
Files records in accessible, confidential and secure system.
Ensures documentation is clear, concise and accurately reflects information provided by the patient to assist the physician in determining treatment plans.
Enters orders into the medical record as directed by the physician.
QUALITY IMPROVEMENT
Supports the quality improvement activities of the facility and responds appropriately to change.
Participates in learning or teaching experiences as appropriate.
Requests supervision when needed and utilizes supervision effectively.
Ensures compliance with policy, procedure and regulatory requirements.
Maintains patient rights, confidentiality.
Ensures appropriate documentation for billing, if applicable, is completed to maintain ethical billing practices.
-Minimum Associates Degree in Nursing (RN) required
-Current Michigan RN Licensure required
-BLS For the Healthcare Provider 60 days after hire required
